That depends on the state where it is to be used I believe, but I am not a
lawyer and don't play one on TV, so get a legal opinion if you can. When you
present it to someone, they will evaluate it based on the laws where you
presented it. They generally don't know the laws elsewhere. So it must be
legal there. Some states (and I think Texas is among them) require a notary
seal on it. Others do not.

I had the Unaccompanied Minor form last year. So, I will have it this year
as well. I noticed that there isn't a place on it for a Notary's stamp. Does
this mean it does not have to be notarized?
