Daniel, if a pile were strangulated, a leech would be a good thing to use to reduce the swelling and prevent gangrene.

(speaking from the experience of one who has seen a strangulated pile and the gruesome details of surgery, reconstruction, and subsequent care, including cauterization when the stiches ripped out because the patient was too damn stubborn to rest after the surgery....Yeachhh)
