I think I can take a stab at answering this one even though I'm not actually
in the Order in question...


You see, at every peerage circle they bring a large lottery hopper with the
names of all the people in the kingdom on the ping pong balls.  They roll a
four sided dice to determine how many balls will be drawn.  Each ball is
then announced 'bingo style' and each of the peers in the circle flip a coin
to decide whether they will vote 'yes' or 'no'.  A unanimous vote (in the
affirmative OR the negative) gets the approval of the crown and that person
is then elevated.  You might be curious how a person with a unanimously
negative vote would become elevated... simple: if they did not do this,
there would never be anything to shout about.
