Speaking only on the Queens Field from my perspective....

No, the "best" fighter does not always win.  Some drop at quarters or Semis.
  Some are in just to knock a particular combatant out, but this is not very
common.  Aside from this, the resulting Queens Champion is normally one of
the top 10 fighters in the Kingdom. A dark-horse may appear and get as far
as quarters but is normally knocked out by Semis or Finals. Queens is the
Rapier Community's "Crown Tourney".  By Quarters in Queens, Combatants who
are not "going for" Queens Champion normally bow out and withdraw from the
list or ratchet down their fighting as to be knocked-out quickly.  It is not
unusual for 5-10 fighters to withdraw.

A typical Queens list ranges from 40 to 50 even as high as 100.  One needs
only to compare this to the number of entrants in recent Crown lists.
