Cervical spine protection down to the first thoracic vertebra, for one. 
Hands has to include the wrist bones. Shoes.
The Ansteorran regulations add the following:
- a wider chinstrap if leather is used instead of nylon (penalizing period 
materials)
- forearm protection (with some rather picky coverage requirements)
- additional padding on the gorget
- additional padding on the kidney belt
- additional padding on the demi-gauntlet if rigid material is used that 
does not transfer the force to the haft
The latest version onfthe SCA regulations specifies a lot of padding under 
many types of gauntlets and demi-gauntlets,  does not accept a shield alone 
as protection for an elbow, and requires padding under camails and rigid 
gorgets.
