Note the Judging form page where it says.

    * *We don't have a separate "entry" form for competitions.  The
      judging form is the entry form.  If a brewer would like to print
      out judging forms for all their entries, they are welcome to do
      so.  Or they can simply turn up at the competition and fill out
      the form on the spot.*

    * *_Because "documentation" is required with each entry, brewers are
      welcome to download the MS Word version of the judging form, and
      then add their documentation information directly on the form
      before printing it and bringing it to the competition. _ *
