Different people see different connotations.  In many cases, I see
"macho" and "machismo" to involve "excessive masculinity" (supra.), or
rather excessive displays of what the bearer fondly believes to be
masculinity.  So, personally, in many cases I tend to see them as mild
insults: that he's either excessive, foolish, or bullying.

My cow-orker helpfully points out that it depends on context.
"'Macho' is not something that I'd like to be applied to my dentist."
