He was called "John the Plain" because when people asked him his name, he would say, "John." They would invariably counter with "John What?" to which he would reply, "just plain John." So he became known as John the Plain. He was John the Plain of Shearn because he lived on Shearn Street in Houston. "John-John" was a term of endearment. You can work out for yourself where "John-John the Plain-Plain" came from.
