[Ansteorra] Award Form Prototype

Greetings Ansteorra!

One of the issues discussed this past Round Table was a need to improve our
award recommendation form. Taking into mind the commentary given there, I
have constructed a prototype that I think will solve some of the issues.

Please keep in mind...THIS IS ONLY A PROTOTYPE! It is neither perfect in
form or function but will merely give you the sense of where we can make
submitting an award a much more efficient and enjoyable process. Feel free
