[Ansteorra] Award Form Prototype

It worked for me but I don't see that it is that much different from what
we already use. Except for the others who support which is more like what
we had before this system...which I liked better than this one we use now...

One thing I liked about the original one before this one is it was
*decorative*. It made me feel like I was still doing something with pomp
and with ceremony even if it was on-line and on the computer. What I've
disliked about the one we use now is it is so cold...this one isn't any
better on that account...but maybe I'm just an old fashioned fuddy-duddy...
