This is to inform all hospitalers that the new reporting form is currently
online
This form is accessible from https://goo.gl/forms/HuL7bH73g3uztxfF2

When you submit the form you can request that a copy of the results be sent
to yourself.    You will be able to then forward this with your report to
your local groups.    It is hoped this reporting form will streamline the
responses.
