[Ansteorra] Hospitaler officer application

the form does send a copy of the notice of submission but not the full
details.   it does not send to the local group's seneschal or B&B   we have
limited access to server security protocols for a regular web-based form.
google has a lot of security measures to reduce spamming.   one of which is
that it does not allow the submitter to send to multiple email addresses.
Years ago we had a web based form for officer reports but the system kept
getting spammed which is why. the form is now being generated through
google forms.

This form will not be used for the application for the kingdom office
itself as that is sent to crown with a cc to the kingdom hospitaler. this
form is not being used for other kingdom offices as it limits it's
responses to email addresses within the hospitaler's office.
