Although archery has perhaps the best safety record of all organized sports, it is it only so because we all know how seriously dangerous the sport could be if we did not all remain mindful of safety.  Equipment malfunctions and misuse can cause injury.

For instance and overdrawn arrow might cause and injury like this.
