Don't bother....
1) The forms are in Adobe Acrobat Form format. You should be able to fill
them on your computer using the free Acrobat reader or print them out and
fill them out by hand.

2) or, I have the original Word files.

I recommend against Word, as the formatting changes on every machine and as
you change the default printer. That's the beauty of Adobe - it's platform
independent.
