Not exactly, it just makes people adjust their fighting up a few notches in
the skill department if they now have to worry about being wounded (I know I
have done it before where I "sacrifice" the arm or the leg in order to
divert the blade so I can get the killing blow. It also stops the people who
really like and count on the quick one shot to the head or "snipe" attack if
they lose the match that way. I believe that the proper format would lead to
a better display of skill and point control from the fighters, be it first
blood or the first to "incapacitate" their opponent without killing them,
mabey the first to take 2 or three of the opponents limbs, or even the first
to deliver so many "tip cuts"

I do see your point (quick, somebody PARRY that), but my understanding
of those would id that they did not always die quickly, like they would
on the chivalric side but often time linger (sometimes for days) and
therefore would be in need of our services during that time. Also, since
our "art" many times during the better part of the period we study, the
prescribed treatment was a "blood letting", so a first blood tournament
would also seem appropriate to the Sponsorship of the Chirurgeonate.

I disagree. If the opponent is dead then chirurgeons are not necessary.
Only if they are wounded (and desire treatment) would our workload be
overwhelming.
