[Central] Unidentified Waivers from Burleson

I've received a fighter practice waiver (or what was left after the Post 
Office ripped it).

All it says is "Fighter Practice", 5-17-03.  No indication what group. 
The return address is in Burleson, TX.  Can anyone clue me in which 
group this is for?
