Hi,
	You might want to go to the hardware store and pick up some pipe
insulation.  It's already a tube with a slit up the side.  It comes in many
shapes and sizes, and it's rather cheap.
	To hold the foam to the PVC, you might want to try one of the
following:
*	Contact cement.
*	Double stick tape.
*	Anchoring the foam to the PVC on the handle, which will allow one to
directly anchor the padding to the PVC, as opposed to using friction to hold
everything together.
