Sorry for the bandwidth, but I don't have a complete list of those for whom this might be important.

The Gulf Wars policy (and that of Ansteorra) requires Chirurgeons and Waterbearers who go onto an active field (including one for which a hold has been called) to have a non-combat authorization on a standard fighter's card. If you wish to act as a Chirurgeon or Apprentice at Gulf Wars, then you should find an authorizing marshal (the rules do not specify a particular kind of authorizing marshal for the non-combat authorization). Note this requires you to sign the wavier on the card. You will need to know basic rules of the field, but most marshals are glad to tell you what you need.

If you do not have such a card, you may still act as a Chirurgeon or Apprentice to a warranted Chirurgeon, but you cannot enter the field to help an injured fighter.

To apprentice you must (1) choose a warranted Chirurgeon with whom to apprentice, (2) present your first aid certs, CPR certs, and SCA membership card to that person, and (3) fill out an application to apprentice and give it to your warranted Chirurgeon (who will complete the form and file it for you). 

Until you are warranted you may only act as a Chirurgeon under the observation of a warranted Chirurgeon. How much observation must be done by the Chirurgeon to whom you apprenticed and how much may be done under the observation of other Chirurgeons is negotiated with your apprenticing Chirurgeon. You are asking them to vouch for you and they need to feel very comfortable with you and your knowledge of SCA procedures.

Each time you act as an apprentice the observing Chirurgeon must fill out a form and send it to the Kingdom Chirurgeon. It is an evaluation of your work. The Kingdom Chirurgeon will use these and the recommendation of your apprenticing Chirurgeon to determine when you may be warranted. You should print several of these forms and keep them with you in case the warranted Chirurgeon does not have them. In fact, you should print several copies of the event forms (short and long).
