It is raised embroidery or padded embroidery depending of what you 
read. It also uses a small wire to help in forming the figures and 
pushing out the pieces of embroidery. They are a lot of examples in 
books on Stumpwork showing caskets from the 1400s and 1600s. If you 
interested look at:

Stumpwork: The Art of Raised Embroidery by Muriel L. Baker

Stumpwork: Historical and Contemporary Raised Embroidery by Muriel 
Best

Both are out of print but your local library should be able to find a 
copy for you thru inter-library loan.
