HOWEVER.

You should talk to a herald before filling it out and making copies.
There's a box known as "The Box of Death", the implications of an
authenticity request should be explained (it's requesting EXTRA
CHANGES), writing name documentation is a reasonable subject for an
hour-long class, et cetera.  So, yeah, going to your local herald can
be a good thing.  (Or Eirik, or me, or Mari ingen Bhrhihahihnhhhh, or
whomever).
