I do not have a standard format to hand.  I would suggest giving the
title of the page, the URL, and the date accessed:

    "Beyond Kastenrazi: Obscene Norse Nicknames", http://www.sca.
    org/laurel/namestuff/wiggle_arse.html, 27 March 2001
