You have to purchase those from ecclesiastical authorities.  Heralds do not
deal in them.

Yes, it is a place-name.
Bardsley, Dictionary of English Surnames,
page 256 header Duffield gives a 1556 marriage of John Duffell.  There are
other spellings in Reaney & Wilson
as well but this is closest one.  It is also de Duffeld
and may be van Duffeld in Holland.
