Just the standard badge submission form.

If the baronial award *name* is not registered, you should probably
submit it at the same time (and put its name on the badge form on the
"Name this badge is to be associated with" line).  To submit a
baronial award name, use "Name Submission by an SCA Branch".

Wherever you find any other submission forms.  Local heralds should have
master copies sent when the new forms came out lo these long aeons
