The letters are all just examples.  They often have to be modified to
fit the case under consideration.  The essence of each of the letters
is along these lines:
- identifying all parties completely, which means SCA and/or real
  names
- one or more parties give certain rights or permissions to other
  parties
- these rights or permissions are expressly stated, not implied
- where applicable, the grantor(s) state that they realize that at
  some point it's irrevocable

I'm afraid I can't think of how to express it better than the form
letter in Appendix D, just modified to refer to the name rather than
armory:

    * Permission to Conflict

    I, [Name], known in the SCA as ([Society name]) give [Name of
    submitter], known in the SCA as ([Society name of submitter])
    permission for [his/her] armory "[Blazon of submission]" to look
    similar to, but not identical to, my armory, "[Blazon of
    registered armory]". I understand that this permission cannot be
    withdrawn once [name of submitter]'s armory is registered.

    [Date] [Signature of [Name]]

Suppose that a guy with the real name Billy-Bob Bloggs wants to
register Daniel of Lincoln.  Suppose I give permission on June 20,
2004.  I would send this letter:

    I, Tim McDaniel, known in the SCA as Daniel de Lincoln, give
    Billy-Bob Bloggs permission for his SCA name Daniel of Lincoln to
    be similar to but not identical to my SCA name. I understand that
    this permission cannot be withdrawn once Billy-Bob Bloggs's name
    is registered.
