Two more notes, then.

The CoA Administrative Handbook Appendix D is "Suggested Standard Form
Letters".  The first is "Permission to Conflict".  It's a strong
suggestion that you use that: other wording can work, but using the
standard means that nothing that Laurel considers critical will be
left out, and Laurel will not have to contemplate whether a more
customized letter is acceptable.

Note that the letter has to be on paper, signed by the donor, one copy
per copy of the applicable submission form (the device form, in this
case, so, what, ten copies?).  They can sign one and you photocopy the
rest.
