[ANSTHRLD] Court Report Form

All, 

Be aware that the form consists of two pages. 

Additionally, there is a check box when the award was announced but the recipient was not present. The Crown can choose, and usually does so, to date the award when they intend to recognize an individual. This lets me know to not enter the award into the database until I get later notification that the individual received the charter. 

As an aside, local heralds need to let me know if such an event happens.
