I require that you pass that form through my office, since it is the 
responsibility of Zodiacus to manage all things related to the Order of 
Precedence.  I, in turn, will consult with Star Principal regarding any 
automatic system.  Until such time as Star and I have reviewed any automatic 
form, it will not be provided onto the Kingdom server.

At this time, I will not accept digitally signed unless only executed by 
Golden Staff, and with the Crown's concurrence.

I just looked online adobe acrobat 9 can create drop down boxes in forms.
I dont own a copy of acrobat but i will see if i can create the form in
libre officr.  A program like fill and sign can handle the data entry and
signature.   The form once created can be put on the heralds site for
everyone to download.   Then it is just a case of running form. Having
court herald and crown sign form and emailing it off.
