Withycombe is discussing English use (indeed, she is not reliable when
describing name use outside England).  As for use as a girl's name, I
think that's referring to use *primarily* or *exclusively* as a girl's
name.  If you check the introduction for men's names used for women,
you should see what I was writing about before, the use of men's names
by women in England.
