[ANSTHRLD] Submissions Rate

This information differs from what is actually on the 2006 submission forms:

For name submissions, 2 copies of the form and documentation are to be sent to 
Asterisk, 1 copy is to be retained by the branch herald, and 1 copy is given to 
the submittor. For device/badge submissionsu, send 3 colored and 1 non-colored 
copies to Asterisk, 1 of each is retained by the branch herald, and 1 of each is 
given to the submittor. All money and all submissions go to Asterisk Herald.

On the forms,
Names: 3 copies to Asterisk
Devices/Badges: 4 colored and 2 line art copies to Asterisk.

Do we need to get these forms updated?
