I figured out a way to create simple heraldic submission line art forms

Programs needed
cutepdf If you want to save a pdf of the output.
adobe reader
paint program or scan of artwork

Create artwork line art only

Go Into adobe and fill out form.

Attach signature

choose image as signature

Choose image of artwork

Scale to proper size

Print form
