Connie is right there is no form just carry your receipt, it can be hand 
written, to the tax office and the old tag if it has one, and they will ask 
a couple of questions and then they do the rest.

You need only two tools: WD-40 and duct tape. If it doesn't move and it 
should, use WD-40.
If it moves and shouldn't, use the tape
