There are four kinds of entries at KAS:  

Static Entry - a physical item, like a ceramic pot, a basket hilt, a plate of sausages, a shirt, etc., with written documentation.

Scholarly Paper - a paper about a topic you have researched, like the English strategy at the Battle of Crecy, or how astrolabes work, or the history of the spice trade.

Performance Entry - a performed song, story, poem, dramatic piece, instrumental music piece, dance, etc., with written documentation.

Supported Research - this one is a bit different.  Sometimes when you research something, like a metalworking technique, or trying to make sense out of a half-rotted garment out of an Irish bog, you have to make the item in order to figure out how the process works or whether the garment's accepted interpretation as an adult's garment is feasible.  In this case, your work, your actual entry, is the researched paper you have produced, but the physical item you made to support your conclusions needs to be presented as an illustration.  It's like the physical item is an appendix to your paper, but is not what you are entering.

You will find that, just as in the search for ultimate truth, there are many ways up the mountain, which is to say, there is no one way to write documentation.  Ask a Laurel, because part of a Laurel's job is to help artisans with documentation.  Better yet, ask more than one.

The kingdom judging forms are also on the Laurel' page.  Everything the judges will be looking for is in the form.  
Don't fret if you didn't use all period materials or techniques or equipment.  Just say why you didn't: period materials aren't available, don't have access to period tools, have a physical condition that prevents my using period techniques, the period component is toxic, etc.  And "My budget required modern substitutions" is a perfectly legitimate answer.  Just make sure your documentation says what the period components, ingredients, techniques and equipment ARE.
