Yes, he signs the waiver.  If he has any other proof of membership then 
he doesn't have to pay the NMS.

If she has a white card she has to sign the waiver.

Assuming the 13 year old would pay an adult fee.  Some events are 
discounted for teens.  If teens are discounted, then no NMS.
