  The book is _Fabulous Feasts: Medieval Cookery and Ceremony_ by 
  Madeleine Pelner Cosman.  It was originally published in 1976 by
  George Braziller Inc., 60 Madison Avenue, New York, NY 10010.  The book
  I was given is the sixth printing, dated 1995.

Throw it far away.
  
  One word of warning before you rush out to buy it:  the lavish documenta-
  tion relates to the facts stated in her general chapters on such topics 
  as "bread," "raw ingredients," "table setting," "Feast Itinerary of Sir 
  Gawain," etc.  INDIVIDUAL RECIPES ARE NOT SPECIFICALLY DOCUMENTED, and 
  she does not even include the original recipe.  Still, I found the book
  fascinating.

It's the second worst book in the Corpus of modern reprints.  (Take a
Buttock of Beef is worse).
