I bought my copy of _CoI_ from Oxford University Press, who publish
it for the EETS.  I reached them by phoning 1-800-555-1212 and
asking for their phone number; I ordered the book by mail.  As I
recall, it arrived quite promptly.

You can also order it through bookstores.
