Depends on which medieval cook your referring to.  There were a group of
people who went from manor to manorr and 'set up shop' so to speak whose
specific job was grinding spices and things.  Also my simple wooden mortar
gets things ground pretty finely.  There are also several places where the
cook is instructed to sift their spices after grinding them.

Of course, the notion that the cook actually did their own spice grinding is
questionable in itself.  In all likelihood, the average person if they could
afford spices, would have bought them pre-ground at the apothecary. Cooks for
large manors and castles would not have ground them themselves.
