I know that in Lakota medicine diabetes has been known for a long time.
There is a blue flower that treats it.  My understanding is that the
medicine person smelled the urine for sweetness and smelled the breath too.
It's hard to document with the native American cultures because of the
wonderful oral tradition, abut I thought that might be of interest.
