The latter is the ceremonial person "in charge" of a barony; the former
isn't. The Barony of Three Rivers, the Barony of Shattered Crystal, the
Barony of Calafia, the Barony of Storvik (in the kingdoms of Calontir,
the Middle, the West, and either the East or Atlantia, I'm not sure where
the border is), for example, all have landed Barons (and Baronesses)
in overall charge. They're installed by the Crown in consultation with
the appropriate populace, and act sort of like mini-Crowns.
It's one of the closest things the SCA has to a true fiefdom and a feudal
structure.

"Court Baron" is mostly an award, granted either on its own or in many
cases to territorial barons or baronesses who've just stepped down. In
Calontir it's often given to people who've performed long-standing
personal service to whichever Crown gave 'em that title (retainers,
in other words, mostly).
