As far as I'm aware, it only means both in North America, and
possibly only in the US. Elsewhere it unambiguously means an
alcoholic drink, hence my curiosity.

While it's not inconceivable that other places have lost the non-
alcoholic meaning, I can't think of any references that would imply
this is the case.
