SIGH, not so easy to serch the catalog and unless you
know the other titles of book (or author?) time
consuming -- sounds like that  might make a creative
project for student research...

Project Gutenburg has "Forme of Cury"  online.  This
is the 1780 
(according to a note) edition/translation by Samuel
Pegge.
