This shows the usual suspects, notably the Tacuim Sanitatis image. There are lesser known, and older, images of bread out there. The ninth century Utrecht Psalter has several images of loaves, though one has to blow up the tiny images to see them:

The French National Library has a number of initial letters from manuscripts on-line, several of which show varied bread images.
