Well, in Calontir, the Seneschals and autocrats have them : )  I'm not Seneschal
any more (thank you, God), but looking on a copy I just happen to have in the
Treasurer's files:

_MEDICAL AUTHORIZATION FOR MINORS_

The SCA _requires_ minor participants (i.e., those having to have waivers)
whose parents or legal guardians are not present at the event to have a valid
Medical Authorization form.  The SCA _recommends_ use of the Medical
Authorization for all minors whose parents or legal guardians are present.

These forms _have_ to be available from corporate, among other things the
Minor's
waivor mentions fencing, so you know they aren't Calontir-exclusive forms <g>
