[Ansteorra-announce] multiple signatures on fighter practice waivers

[Perronnelle Charrette]

If a fighter attends multiple fighters practices in a month, he or she
must sign and date a waiver for each attendance.  Adding another date
beside an existing signature is not allowed.

For minors, a new minor's waiver needs to be done for each practice.  No
multiple dates on one form.
