Still smoking as I pull it out of the heraldic files....The Ansteorran
College of Heralds has an ACHIEVEMENT REGISTRATION FORM.  The one I have in
the Tempio files is dated from 10/93.  Basically it is a simple form that
your local herald "should" have.  It asks such info as:
1)SCA name
2)Persona Time and Place (so as a 10th cent. Dane you don't receive a 15th
cent. Italian scroll)
3)Registraion date for your name and arms(these BOTH must have passed in
order to get your "REAL AWARD" scroll.
4)Real name, address, phone, etc...
