[Ansteorra] TRF forms

I have tried to confirm passes ahead of time but the
address keeps coming back undeliverable. Also, I have a
minor consent waver but no notorized form is to be found
anywhere. Are there two? There is no place to norotize on
the minor consent form I have. Sorry for the posting but I
