Generally in 3 layers. Some claim 2 but I would bet most would be too weak
to pass at two layers within a year. Seems like a lot of time an money to
invest making something that will fail in a year.

If you mean over the fencing mask, yes. The protection worn with actual
helmets varies depending on the helmet.
