Yes and no.  Bruce prec., under Augmentation:

Augmentations in Society armory should always be blazoned as such; the
bearer has the option of displaying the armory with or without
the augmentation, and conflict should be checked against both
versions. (Rondallyn of Golgotha, September, 1992, pg. 26)

On-line precedents are a good place to look for answers for rules
questions -- erm, once you've checked the RfS themselves, and if you
know what to search under.  (I have the on-line prec. as single ASCII
files, which is much easier for searching.)

>From the on-line Admin Handbook, search for "Correction":

B. Blazon and Spelling Changes must be requested if the error derives
from an error on a Letter of Intent. Such changes should be included
on a Letter of Intent for the consideration of the College of Arms
just as if the submitter were in fact requesting a specific change to
the name or armory.

The Admin Handbook is a good place to look for "how can I" / "can I
do" questions.

(By the way, who is it?  I'd like to look at the entry and the LoAR.)

If the Admin Handbook manifestly says you can, then manifestly you
can.  The new "Name Submission by an Individual" form, in the packet
mailing tonight (oh please oh please ph please) has a box labelled
Alternate; I presume the almost-old forms do too, though I don't
remember.  She was misinformed or she misunderstood.

Certainly.  It's a name change.  Indicate on the form somewhere that
she wants to retain the old name as an alternate persona name.
There's no box for that latter on the new forms.

On the new forms:

Society Name: the current registered one (spelled exactly as currently
              registered)
Name being submitted: the new second one
Name type: Primary
Action type: Change
    Also check Other and write in "retain old as alt."
    (Don't take the "(pick one)" business too seriously, I'd say, if
    more than one applies)

Name Documentation: I'd write more fully "Retain old primary name as
an alternate persona name.", and highlight it to be paranoid.


By the way, anything you do with registration cannot revoke awards.
You the person get them, not a persona.

As for the OP, the outward and visible sign (other than your charter)
of your award: I suspect that few OP heralds read each LoAR looking
for name changes.  I suspect that fewer would alter their databases to
match.  -- That's actually an interesting philosophical question.  If
the Crown gives an award to Foo Bar and he later changes his name to
Baz Quux, should the OP be changed?

Since she moved in, she should write a letter to Zodiacus with proof
of her awards -- copies of pages out of her kingdom newsletter, copies
of her charters, whatever Z. says he wants.  He's at drkook255 at aol.com
if you want to ask -- or so say the latest roster I have; confirm in
the AG.  When she changes her name , she (or her herald) should write
Z. another note with the name change, just to make sure.

Everyone should check their OP info.  There are a lot of gaps.

I hope inquiring minds have noted the research locations.  -- Not that
I haven't been lazy and posted a question that 15 minutes of research
might have answered, in the hopes that someone else might know off the
top of their head.  And not that this list has a lot of traffic.
