[ANSTHRLD] Polish name help

I'll probably kick this over the scahrlds list, but figured I'd try here
first.

We had a submitter at the Coronation consultation table who wishes for a
Polish name.  We found documentation for part of it, but not for the
surname.

He wants "Martyn Lucas Polska".

Martyn and Lucas are both documented from "Polish Given Names in.." at
http://www.s-gabriel.org/names/walraven/polish/

I do not know, however, if double-given names are appropriate at all for
Polish.

His only documentation for the surname "Polska" was a photocopy of a
book cover (not the publication page, alas), entitled "Szlachta Polska.
i jej Pa{n'}stwo".  Subtitled/written by "Muzeum Tradyeji
Niepodlegloscionwych w Lodzi ul. Gda{n'}ska 13".  Help?
