Permission notice?  Such as me saying "Yes that is fine to leave me
there?"

Something like:

I give my permission to use the following contact information:
<Your contact info here>
<your name here>



Yes I need to make sure that everyone on the contacts page I have a
email saying that I have their permission to put their info on the
website.
