Double surnames were used in some places at some times before 1600.   
In England, the practice emerged in the late 16th century and  
remained very rare through the end of period.  I am not aware of any  
instances in which the surnames were conjoined with hyphenation.

There is some discussion of the earliest English double surnames in  
the introductory material to Elizabeth Gidley Withycombe's _The  
Oxford Dictionary of English Christian Names_.  Portions of it were  
quoted in the LoAR of July, 1992 <http://heraldry.sca.org/loar/ 
1992/07/lar.htm> and February, 2002 <http://heraldry.sca.org/loar/

Some additional instances of the pattern in period names were cited
